Medical Case Management-Family Centered Services
Our case management services are a part of a comprehensive family-centered program that provides peer support, creates connections between care seekers and caregivers, and promotes the participation and empowerment of women, their families, and members of their support network in developing care plans based on their self-defined needs. We accept, as well as provide, referrals for women, women and their children, to/from other programs that provide local primary medical care, support and mental health services for women with HIV/AIDS either currently not in a system of care or who need support to remain in a system of care. We provide services for women and their families that assists them in accessing respite care, childcare, housing, food, transportation services, etc. providing women with opportunities to take care of not only their own primary medical and psychosocial needs but also the needs of their children and other family members. Prevention with Positives (PWP) Program Enhancing the health and well being of women living with HIV/AIDS while reducing their risk through multiple interventions that include comprehensive risk counseling and services and the CDC effective behavioral intervention (EBI) Healthy Relationships. Coffee House Support Group The Coffee House support group, ongoing for almost twenty years, provides a safe space for women living with HIV/AIDS to gather, share stroies and resources, and set a course for their lives through building sisterhood and enhancing their knowledge of treatment and care. Find out the details here. Peer-Based Support and Skills-Building We sponsor peer-based support and skills building groups. Our support group model is built upon shared peer leadership and mutual self-help. These services have been available since the inception of the organization in 1993. Currently care staff are providing the Health Connects program to support women and girls in accessing web-based information on general health, women's health and HIV/AIDS. Groups are open to all women and girls regardless of HIV serostatus. Family Social Activities TWC is committed to providing activities to support women and families in creating stronger social networks and offer an opportunity for women to appreciate activities that are not HIV-related but increase their sense of self and connection with others. GED & Literacy Training Women are offtered onsite support and training in thier efforts to increase their literacy and move towards completing high school degrees. Community Kitchen & Pantry Onsite women have access to a community kitchen with light meals and snacks which are made available to women, many of whom do not have access to or the resources for providing themselves three meals per day. Our Pantry provides women and families with basic groceries on an emergency basis. The Tiffany Fund The TWC Tiffany Fund is an emergency assistance program which provides rental, utilities, telephone, medical/dental care, medications, shelter and meets the basic needs of women living with HIV/AIDS and their families. The Tiffany Fund was established in 1998 in honor of Tiffany Nalls, daughter of our founder/executive director Pat Nalls, who lost her life to AIDS at the age of three. The Tiffany Fund helps women and their families to access crisis intervention services and provides direct and immediate financial assistance for emergency short-term needs while permanent support, often through entitlement programs, governmental programs or other means is secured. The Women’s Collective serves women living with HIV/AIDS and their families by processing Tiffany Fund emergency assistance applications in a respectful, confidential manner through a process that retains women’s dignity and addresses the complex emergency needs of families for which, often times, emergency financial assistance is not readily available. |
